The Challenge

What was at stake

In the regions the NGO serves, patients often travel hours to reach a clinic, only to find that the available physician doesn't speak their language or specialize in their condition. Internet connectivity is unreliable — 2G in many areas, with frequent dropouts. Existing telemedicine platforms assumed stable broadband connections, were English-only, and had no mechanism for consultations when connectivity was interrupted. The platform needed to work in conditions that most software is not designed for.

Offline-First Architecture & Connectivity Optimization

Built an offline-first mobile application using SQLite for local data persistence and a custom sync engine that batches and reconciles data when connectivity is available. Video consultations automatically degrade to audio-only, then to asynchronous voice messages based on detected bandwidth. Implemented aggressive data compression achieving 80% reduction in bandwidth requirements compared to standard video consultation platforms.

AI-Powered Multilingual Communication Layer

Integrated Whisper AI for speech-to-text across 12 languages and built a real-time translation pipeline that translates between patient and physician languages with medical terminology awareness. The system handles code-switching (patients mixing languages) and provides physicians with both the original transcript and translation to verify context.

Community Health Worker Integration

Designed workflows that leverage community health workers as intermediaries — the health worker's device facilitates the consultation, handles technical setup, and assists with language nuances that AI translation might miss. Built a CHW toolkit with guided assessment protocols, vital sign recording, and triage decision trees that work entirely offline.

Clinical Quality & Outcome Tracking

Implemented a clinical quality framework with consultation outcome tracking, follow-up adherence monitoring, and longitudinal patient health records that persist across providers. Built analytics dashboards for the NGO showing consultation volumes, disease patterns, and health outcomes by region — enabling data-driven decisions about where to deploy resources.
